Dislocated iud...help.?

i have a dislocated iud, and i had an appointment for an ultrasound but i never went for fear of the removement process and because i no longer have insurance. i read an answer that said if the iud is dislocated, they remove it and replace it with a new one. my question is how do they remove it? surgery? what type of procedure?
