What is physically wrong with my shoulder?

I use to be a physically fit guy that would lift weights swim and do sprint workouts. Not too long ago it seems I have lost my ability to do anything with my arm. I had two surgeries on my shoulder; both were arthroscopic surgeries to help with grinding and pain. Around 2009 I had surgery on my shoulder to help with a minor dislocation that would occur on my shoulder; I was able to rotate my shoulder with my arm bent in a 90 degree angle where the knuckles are facing up and the nails are facing the ground. (Hopefully I described it well enough to where everyone understands) When I rotated my should I was able to dislocate my shoulder with some pain that grew worse and pop it back into place. It started to become harder and harder to lift weights so that’s when I went into surgery. Shortly after surgery I gradually go into the pool for 3-4 months to help with rehab and afterwards got into weights again and felt great. a year passed and then all of a sudden I went to max out my bench an felt a pop in the shoulder I had surgery on. A week passed by after maxing on bench and I tried to warm up on bench and my right shoulder could not even lift 145 pounds and it was hurting. I took a 15 min break and tried again, this time I could not lift 100lb on bench and it gradually dropped to where I was barely able to lift about 45lbs on bench. I took a year off of any physical activity and only stretched my arm, did wall pushups and did as many pull-ups as possible because I had no pain while doing pull-ups. The pain got worse to where it was hard to sleep and cold weather was painful, (I am negative for all arthritis the doctor has told me that) So i went into surgery again on the same shoulder and the doctor was shocked about the story I told him, so instead he went in and trimmed at some of the collar bone where it connects at the shoulder to help with the inflamation and cleaned out where the ball and socket is (The first surgery was to rebuild the shoulder by tightening it up). It has been about 2 months since I had surgery and it seems that the shoulder is going back to before I had surgery. I need advice, I miss weightlifting so much that it is hard to just keep stretching and not able to do much with my arm. I cannot throw a ball; I cannot do any form of pushups besides the wall pushups. It is extremely hard to work out my shoulder by shoulder workouts. I cannot do no more than 45lbs on bench before it hurts. Swimming is also a little painful but goes away after 200 meters in the pool. If anyone knows that would be awesome. Thank you for the help.
